Bachelor's degree in Pharmacy from accredited school of pharmacy and completed state required internship.
Current Michigan license as a Registered Pharmacist.
One to two years' work experience in a hospital setting preferred.
Hospital pharmacy residency desirable
Advanced education such as Doctor of Pharmacy degree desirable.
Supports the concept of pharmaceutical care, which involves pharmacists' cooperating with patients and other professionals to design, implement, and monitor medication therapy to achieve planned treatment outcomes for the improvement of the patients' quality of life.
Accurately dispenses routine, emergency, floor stock and parenteral medications in a timely manner to patients for Munson Medical Center and affiliates/subsidiaries of the Munson Healthcare System.
Accurately interprets and enters the physician orders into the computer, follows formulary control policy and procedures, and provides drug information as requested in a timely manner to patients of Munson Medical Center and affiliate/subsidiaries of the Munson Healthcare System.
Achieves continuing education that focuses on the care for patients in all age categories (birth and above including the unborn).
Demonstrates competency in performance of medication preparation, dosage calculation, and drug information during cardiac and respiratory arrest situations.
Demonstrates competency in pharmacokinetics and renal dosing. Able to assist physicians in other dosing requests.
Demonstrates competency in the use of the departmental policy/procedure manuals.
Complies with federal law, state law, and hospital policy/procedures in the preparation and dispensing of all controlled substances.
Complies with USP recommendations, manufacturer information, and hospital Policy/Procedure in the proper storage, security record keeping, and handling of all pharmaceuticals purchased, repackaged or compounded.
Demonstrates competency in providing basic services to meet the acute care needs of affiliate, home infusion and ambulatory pharmacy patients.
Provides clinical services in patient care environment as assigned or needed.
Demonstrates competency in the use of certain technologies/equipment (i.e., computer entry devices, triple beam, torsion and Mettler balances; the automix compounder, the heat sealer and laminar flow hoods).
Demonstrates competency as it pertains to department safety procedures, such as disaster response, fire safety, bioterrorism, TB prevention, blood borne pathogen precautions, security and waste disposal.
Demonstrates competency in aseptic technique as it pertains to the preparation of parenteral products.
Demonstrates competency in the procurement of routine or emergency medications outside of normal business hours (i.e., afternoon shifts, weekends and holidays).
Demonstrates competency in application of various communication skills as patient counseling, education, drug information, etc.
Completes special project work as assigned.
Performs all other duties as assigned.
