Clinical Supervisor

Job Details:


RN with licensure in Michigan. BSN required. (Equivalent combinations of education, training and experience may be substituted for this criterion.) Minimum of 2 years in home care or hospice as appropriate. Supervisory experience required. Able to communicate effectively in English both orally and in writing. Demonstrated proficiency in planning and conducting presentations for staff, physicians or community members. Possesses current Michigan motor vehicle license, ability to drive a car, and insured vehicle capable of transport to patient homes or other offices and a working telephone in the home. Meets MHH established requirement for proof of health status. Knowledge of appropriate Conditions of Participation and reimbursement required; CHAP standards, denials, and queries preferred. Ability to physically move, position and transfer clients up to 200 pounds utilizing good body mechanics. Required to be a self-starter with the ability to work autonomously with minimal supervision. Demonstrated computer proficiency in windows and word processing applications. Able to operate standard office equipment. BLS-C required.


Responsible for all services provided to a defined group of patients including the following: v Oversees and directs the clinical activities of the field staff, including performance evaluations. v Leads regularly scheduled team meetings. v Conducts team and individual case conferences. v Maintains clinical knowledge base of current patient population. v Coordinates new client referrals, assists with scheduling and problems related to staffing, assures continuity of care in the home setting. v Performs annual performance evaluations, hires and disciplines employees. v Performs admission, concurrent, and discharge record reviews according to established department guidelines focusing on patient care needs and regulatory requirements. v Identifies and addresses aberrant practice trends both in the field and in documentation. v Works with payers and case managers to negotiate visits, rates, reporting clinical updates as required and referring to other departments as appropriate. v Coordinates coverage of job responsibilities for employees taking vacation, sick time, and medical leave. Participates in quality improvement process including the collection of data and reporting as directed by the Quality Manager. Complies with MHH and department policies, procedures and guidelines.
