Job Summary: The RN Clinical Quality Manager will be responsible for overseeing the continuity of care program initiatives and related quality and efficiency metrics, primarily for the Phelps Physician Practices. Responsible for enhancing the quality of patient care and reducing the overall health care costs for patients of Phelps Physician Practices. Manages the coordination of the clinical and reporting aspects of the CPCI program; patient navigation and other care management needs of the Phelps Physician Practices. Identifies and ensures care coordination for the high acuity patient population. The Manager will also identify areas for performance improvement, initiate and monitor follow up plans.
Job Requirement: Bachelor’s Degree required. BSN preferred. MA/MS in clinical field or health administration or relevant advanced training preferred. Excellent verbal / written communication skills, organizational and analytical skills and the ability to plan and meet deadlines required. Ability to work with simple statistics and performance improvement metrics required. Computer proficiency and knowledge of MS Office software (Word / Excel / PowerPoint and Outlook) required. Ability to work and lead teams required. Ability to read, write and communicate in English required. Minimum three-five (3-5) years’ experience working as a licensed registered nurse working in primary care, home care or case management with two (2) years supervisory experience. Current/Valid NYS license as a Registered Nurse required.
