Riverside Shore Hospice is currently seeking a labor pool Social Worker.
Riverside Hospice believes that when time is of the essence, quality of life is essential. Our trained Hospice care providers help reduce the distraction of managing pain or medications so patients can focus on what's important: time together with loved ones. In the comfort of home, patients and their loved ones will receive physical, emotional, social and spiritual care during their journey. Help and support is available 24 hours a day, 7 days a week. Hospice services include: Nursing services, medical social workers, nurses and aides, physical therapy, occupational therapy, speech therapy, nutritional counseling and much more.
Job Summary: Works under the supervision of the physician in accordance with the plan of care to provide quality therapeutic services for patients, residents, and their families. All services are provided by a qualified Social Worker (MSW). Medical Social Services include: assessing emotional factors related to terminal illness, assessing the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect. Assessing environmental resources and obstacles to maintain safety, assisting the physician and other IDT members in recognizing and understanding the social/mental stress and/or disorder that exacerbates the symptoms related to the terminal illness. Participates in the development or revision of the plan of care. Provides social services including: short term individual counseling, crisis intervention, assistance in providing information and preparation of advanced directives, and transfer of responsibilities regarding fiscal/legal and healthcare decisions. Assesses need for counseling related to risk assessment for pathological grief. Assesses special needs related to cultural diversity including communication, space, role of family members and special traditions.
Education: Master's Degree in Sociology/Social Work. Program must be accredited by the Counsel of Social Work Education.
Licensure: Must have and maintain: a current valid driver's license with proof of satisfactory driving record and history; proof of auto liability coverage as required by Virginia State law. Must have reliable transportation to make visits to patient's homes each day.
