PACE, Program for All-Inclusive Care of the Elderly, is a community-based program which provides patients with all necessary health services while they stay in their homes. It covers many different types of services including a day center, personal care in the home, Lifeline emergency response system, transportation to appointments, delivery of medical supplies, therapy, as well as inpatient services.

JOB SUMMARY: Works under the general supervision of the Site Director or Operations Manager. Is a member of the PACE interdisciplinary team and is required to attend meetings on a regular basis. Responsible for the following: assessing the nutritional needs of participants to include initial and on-going assessments annually or more frequently as required by the participants current medical condition. Presents assessment information at scheduled IDT meetings. Is responsible for coordinating nutritional interventions with other members of the interdisciplinary team, developing nutritional care plans and providing clinical consultation and direction to the Food Service Department. Additionally ensures compliance for documentation and nutritional services as required under federal, state and regulatory agencies. Provides nutritional counseling to participants and family members. Provides nutrition education as indicated to the participant and/or designated caregiver, either individually or in group at the day center facility to PACE participants/family members at the PACE facility.

EDUCATION: Bachelors degree in Nutrition or related field as accredited by C. A. D. E. or ACEND required

LICENSURE: Current registration with the Commission on Dietetic Registration required. Must have current, valid driver's license and acceptable driving record and maintain throughout employment. Must have reliable transportation to visit patient homes. CPR Certification and First Aid training required and must be maintained current throughout employment.

EXPERIENCE: 5 years previous experience in health care or geriatric nutrition services preferred; one year experience working with the frail elderly population required.

HOURS: Available to work a varying schedule Monday-Friday between the hours of 8:00am and 5:00pm.

ESSENTIAL PHYSICAL REQUIREMENTS: Local travel required. Must be able to move about the office or facility and stand for extended periods of time (up to 6 hours per 8 hour shift). Must be able to push/pull/carry/lift or otherwise move objects up to 20 pounds occasionally (up to 1/3 of the shift) and up to 10 pounds constantly (up to 2/3 of the shift). Must be able to reach above shoulder level, kneel and climb a short ladder. Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
