Job Summary: Riverside Tappahannock Home Health is seeking a staff Physical Therapist to work various patient care settings. A staff Physical Therapist is supervised by the Home Care Therapy Manager. Following appropriate practice, a therapist will perform evaluations, determine PT diagnoses, develop a plan of care, and perform physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability. This position may also perform other assigned duties. A staff Physical Therapist must demonstrate the knowledge and skills necessary to provide age, goal, and diagnosis appropriate care. The individual must demonstrate knowledge of the principles of growth and development throughout the life span, be able to assess patients' needs and status by analyzing evaluated data, and be able to interpret information to identify age-specific needs and provide appropriate care accordingly per patient care setting. This position must also be able to follow and adhere to the practices described in the Department Policy and Procedure Manual. Services provided by the PT include: initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate time points; developing and revising the plan of care in consultation with the physician and other interdisciplinary team members; providing therapeutic treatments and evaluation of equipment needs as applicable; advising and consulting with the family and other agency personnel as applicable; preparing clinical and progress notes; evaluating outcomes of care; supervising the LPTA and Home Health Aides as appropriate; participating in in-service programs and case conferences; and planning for discharge of patient from the agency. Participates in facility meetings, departmental meetings, in-services, educational programs, and other meetings as required to include solutions and engagement with resolution processes.
Education: Bachelor's Degree in Physical Therapy required.
Licensure/Certification: Physical Therapy Licensure in the state of Virginia required. Healthcare Provider CPR/BLS required. Valid Driver's License and acceptable driving record required upon hire and to be maintained during employment. Must also have reliable transportation to make visits to patient homes each day.
Experience: Previous healthcare experience preferred.
