The Universal Care Partner works under the general supervision of the Senior Nurse Manager with direct supervision by the Recreation/Activity/Day Center Manager, Day Center Manager, Home Care Manager or Home Care Coordinator. The position works to provide care and service both in the PACE Day Center and in the PACE participant?s home which support the social, physical and cognitive functioning of seniors with impairments. These services include activities of daily living, restorative care, supportive care and recreational activities. Assists with the organization and implementation of the activity program. May accompany participants during bus/van transportation as needed and requested. The UCP demonstrates the knowledge and skills necessary to provide an/or assist with appropriate care to include: personal care (feeding, grooming, bathing, toileting), home services to include laundry, memory stimulation activities (social, recreational), motor activities, escorting to medical appointments, shopping, assisting with transportation of participants to and from the facility utilizing the facility transportation vans and other activities as directed by the Participants plan of care (POC). Monitors health status and provides a safe environment for participants. Assists with any identified needs of the Participant during their onsite stay at the PACE facility and in their home that will enable the Participant to perform at the greatest level of independence possible. Functions as an Interdisciplinary Team (IDT) member. Supports and lives out the Riverside Care Difference.
EDUCATION: High School graduate or GED equivalent preferred. Must also have proof of the following: A) Holds a certificate of completion from a State of VA approved nurse aide training program and can be registered by the Virginia Department of Health Professions as a certified nurse aide or B) Hold a certificate of completion from a professional nursing practical nursing or nurse aide training program approved by the Virginia Department of Education or the Virginia Community College System or the Virginia Department of Health Professions or C) Holds a certificate of completion from a home health aide training program approved by the Department of Health prior to July 1, 1984 and have been in continuous practice since that time.
LICENSURE: Licensed by the State of Virginia as a Certified Nursing Assistant (CNA) required. Proof of current, valid drivers license and acceptable driving record required upon hire and to be maintained during employment. BLS for Healthcare Providers Required.
EXPERIENCE: Previous the field of Personal Companion, Personal Care Aide, or Nurse Aide preferred. Must provide proof of minimum one (1) year experience working with the frail and/or elderly population in any capacity.
ADDITIONAL REQUIREMENTS: Ability to move about in the home and facility and stand or sit for extended periods of time. Ability to lift, push, stoop and pull in order to ambulate/transport patients assisted and unassisted. Must be able to exert up to 20-50 pounds of force occasionally and 10-25 pounds of force frequently and up to 10 pounds of force constantly.
HOURS: Ability to work varying shifts (day shift, evening shift and weekend shift) based on Day Center and participant's home care needs.
