We have an exciting opportunity for a LPN. This position works under the general supervision of the PACE Clinic Manager. The position is primarily responsible for general nursing care of the PACE participants at the Center and /or facility clinic and provides care as identified in the PACE Participant's individualized plan of care to include documentation to the electronic medical record. Supports clinic activities, administers medications and treatments to participants, monitors response to medications and treatments and reports significant findings to RN Care Manager and/or provider. Responsible for documentation of medications given in the participant's MAR. Responsible for monthly change out and verification of current months MAR. Demonstrates knowledge and skills necessary to
provide age appropriate care for the frail and elderly population according to assigned responsibilities and Riverside PACE standards. May also be required to provide care, training or education to Participants in their home on an as needed basis.
Provides feedback and recommends changes in participant's status to PACE clinical team and participates in case conferences as indicated.
EDUCATION: Graduate from an accredited school of nursing with diploma in Licensed Practical Nursing required.
EXPERIENCE: At least one year LPN clinic experience required, three years preferred. Home Care or Skilled Nursing Care experience also preferred. Must have minimum of one year experience working with the frail or elderly population.
HOURS: Monday through Friday 8:00 am to 5:00 pm primarily, additional hours to meet the needs of the facility including some on-call responsibilities.
