Riverside PACE MacTavish in Richmond, VA is currently seeking a full-time Social Worker MSW.
Riverside PACE is a Program of All-Inclusive Care for the Elderly serving seniors who live in metro Richmond and on the Virginia Peninsula. PACE is a community-based alternative to institutional long-term care. PACE empowers participants to remain in their homes by providing comprehensive medical and social services. The program enhances quality of life for family members and caregivers through support services and peace of mind.
Job Summary: Completes biopsychosociospritual assessments of current and potential PACE participants. Works collaboratively with the interdisciplinary team (IDT) to develop care plans for participants. Refers participants and/or caregivers to appropriate community resources. Serves as advocate and liaison between participant and PACE staff. Provides education about disease progress and/or facilitates participant/family meetings with appropriate PACE staff for same purpose. Facilitates advance care planning with participants and/or caregivers including completion of advance directives if possible. Assists with crisis intervention for participants and caregivers. Facilitates caregiver support groups. Communicates concerns regarding behaviors, mental health or health status to IDT. Actively participates in daily IDT meetings. Coordinates respite and facility/hospital discharges. Assists with admittance to long-term care facilities. Maintains thorough documentation of case management activities. Serves as resource to PACE staff. Completes appropriate quality control documentation as needed.
Education: Must have a minimum of a Master's degree in Social Work. Program must be accredited by the Counsel of Social Work Education.
Certification: CPR Certification and First Aid training required and must be maintained current throughout employment. Current, valid driver's license and acceptable driving record required upon hire and to be maintained during employment. Must also have reliable transportation to make visits to patient homes each day.
Experience: Must have at least one year previous experience working with the frail or elderly. Two years previous social work experience in a healthcare setting preferred.
