The Home Health Physical Therapist will evaluate, determine appropriate PT diagnoses, develop a plan of care, and perform physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health and preventing or limiting permanent physical disability.  Services provided by the PT include: initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate time intervals; developing and revising the plan of care in consultation with the physician and other interdisciplinary team members; providing therapeutic treatments and evaluation of equipment needs as applicable; advising and consulting with the family and other agency personnel as applicable; preparing clinical and progress notes; evaluating outcomes of care; supervising the LPTA and Home Health Aides as appropriate; participating in inservice programs and case conferences; and planning for discharge of patients from the agency.  Participates in facility meetings, departmental meetings, inservices, educational programs, and other meetings as required to include solutions and engagement with resolution processes. 

Must have Virginia license to practice as a Physical Therapist.  Current valid driver's license and acceptable driver's record required upon hire and to be maintained during employment.  Must have reliable transportation to make visits to patients' homes.  Home Health experience preferred.  Full time day shift hours Monday thru Friday.
