The TOGETHER Program is a psychoeducational program developed by two professors at the University of Maryland (UMD). The program falls under the overall direction of the UMD Project Director and Associate Project Director and the UMD Together Program Manager, and according to a contract between Sheppard Pratt and UMD, TOGETHER is designed to promote healthy couple relationships, economic stability, and mobility, for low to moderate income couples. The program is offered in Montgomery and Prince George's counties in Maryland. The TOGETHER Program operates under a grant from the Federal government and operates in conformity with Federal grant requirements.
Duties involve the provision of case management to TOGETHER participants and workshop administration support services. The Case Manager may assist in marketing and recruiting activities, responding to inquiries from prospective TOGETHER participants and referral resources. The position may involve networking with other community agencies and service providers, at times. The worker will be required to prepare certain reports on a regular basis and attend training and related staff meetings and program events. The position requires ongoing communication with the program's Sites and Case Management Director and other supervisory personnel. The first six months of the grant will include planning, in which the worker will assist in assigned administrative tasks in support of beginning services.
Case management responsibilities are provided by telephone and in-person when needed and include, but are not limited to:
•Screening and providing needs assessments for program participants and conducting intake and enrollment services.
•Providing referrals and care linkage, case coordination, weekly consultation, and follow-upfor participants with community resources and safety-net services that are needed including workforce/career development and potentially domestic violence (DV) services.
•Managing required data collection, according to TOGETHER Program policies and procedures.
•Ensuring that supervisory staff are apprised of any participant emergencies or unusual client situations or problems for which additional and specialized assistance may be required.
•Scheduling and participating in meetings with Workshop Facilitators to discuss participant needs and progress and updating the "CM and Facilitator Meetings Sheet."
Job responsibilities relating to workshop logistics, set-up, support, and preparation include these and similar duties:
•Assembling and providing workshop materials needed for class sessions in a timely manner.
•Attending and supporting workshop sessions at varying locations including transporting materials to class sites as needed.
•Purchasing and arranging for food and supplies to be available for scheduled workshops.
•Ensuring that workshop sites are setup and ready for classes (seating, tables, projector,slides, etc.) and broken down after each class and returned to pre-class set-up standards according to the procedures provided by host organizations.
•Responding to phone and other inquiries from prospective participants and scheduling them for workshops and other services when needed.
•When able provide translation of program documents to Spanish as needed.
A Bachelor's Degree or equivalent in an appropriate field is required. Written and oral fluency in both Spanish and English is preferred. Experience may be substituted for an academic degree and the position may be filled by advanced college students and/or interns in some situations. An ability to work in digital environments with standard business applications is required. The position requires flexibility in work schedule, including evening and weekend hours, and access to reliable transportation. When work-from-home is required, must possess reliable Internet connectivity. Must be goal directed and capable of working independently and as part of a team. Must be able to lift 10-40 pounds and move standard office furniture such as office tables and chairs. Respect for cultural diversity and ability to work with low income populations is important. Sensitivity to delicate interpersonal situations is important as the worker will, under supervision, provide screening for DV and other high-risk matters. Good verbal and written communication skills are required.
