This position serves as liaison between Admitting, outpatient clinics, physician offices, hospital departments (e.g., ER, Surgery, Cath Lab, etc..), and nursing units to coordinate timely and smooth patient placement. Works with Care Coordination department to ensure patient placement at appropriate level of service. Identifies and resolves barriers to timely patient placement. Performs concurrent quality reviews and audits as needed, and communicates issues to the appropriate personnel. Assists with obtaining and communicating discharge planning and level of service information to the Care Coordination department.

Requires an RN or LPN with current Illinois license. Minimum three years clinical experience required. Management experience preferred. Excellent interpersonal and communication skills required. Basic computer word processing knowledge and proficiency. Prior experience in Utilization Review or Quality Assurance is desirable.
