SUMMARY The Care Coordinator works as a member of the health care team in coordinating patient care services across the continuum, while promoting and monitoring the effective use of resources. The care coordinator develops discharge plans that incorporate an assessment of the patients' physical, psychological, social, cultural and age specific needs. The individual matches the patients' level of care with the most appropriate community or facility based program. The coordinator applies utilization review criteria to determine medical necessity for admission and continued stay., as well as, achieving quality and cost efficient patient outcomes. This individual utilizes knowledge of patients' benefits and DRGs to guide the care team to develop cost conscious discharge plans. Must understand federal, state and third party payer guidelines, Has knowledge of external resources, provides education and direction to the patient and/or representative in their discharge plan. The care coordinator role requires a role model, educator, consultant, advocate and change agent and is an active participant in quality improvement initiatives.
QUALIFICATIONS Required: Bachelor Degree in Nursing. Experience: Minimum of 5 years nursing experience preferably in an acute care or managed care setting;2 years prior experience in Case Management, preferred (Discharge Planning and Utilization Review);Current knowledge of federal, state and regulatory requirements guiding case management, preferred Working knowledge of the interface of health plans with Care Coordination, preferred Working knowledge of community and support services, preferred. Strong interpersonal and communication skills; ability to manage, teach, negotiate and collaborate with internal and external resources, patients and families
