Graduate of an ACPE accredited College of Pharmacy
Pharmacy Licensure in State of Illinois or eligible
Responsible for the evaluation, interpretation, and management of patients utilizing Specialty Medications. Works with health care providers and patients to facilitate prior authorizations of specialty medications. Reviews and evaluates orders for appropriateness of specialty medications and works with third parties to assure coverage. Monitors patient's drug regimens and intervenes when appropriate to ensure optimal drug therapy. Provides drug information to the Health System staff, patients and community. Carries out the control and distribution functions ensuring compliance with health systems, state, and federal policies, guidelines and laws. Participates in continuous quality improvement activities and educational experiences in support of departmental philosophy and objectives, as well as Health System initiatives. Responsible for financial oversight of billing and patient charging of services provided in the Specialty Pharmacy.
Graduate of an accredited College of Pharmacy. At least two years experience in health care system pharmacy preferred. Direct experience in Specialty or Ambulatory Care Pharmacy Practice required. Completion of PGY-1 Pharmacy Practice Residency or equivalent experience.
Must provide copy of diploma/degree for highest level of education obtained.
Current licensure from State of Illinois or license eligible.
Successful completion of annual clinical, age and job specific competencies and skill verification tools required. Great interpersonal, organizational and critical decision-making skills required. Ability to use calculator and computer. Ability to effectively communicate and develop collaborative working relationships with patients, medical staff and the public.
