Position Summary: To assist in the assessment and treatment of biopsychosocial needs of patients of all ages (birth to death) throughout hospitalization. To provide optimal continuity of care between the hospital and that setting to which the patient is discharged through planning and coordination within interprofessional framework, both inter- and intra-agency. Screens the patient for financial needs and assists with connecting the patient to area resources, as needed.
Qualifications: 4-year/Bachelor's Degree in Social Work and current license as a Social Worker in SC required. 1-3 years of experience required. Must be computer literate. Must have high level of confidentiality.
OSHA Category: Rare exposure to risk of bloodborne diseases (Category II). Rare exposure to risk regarding chemical hazards (Category B).
Physical Requirements: Exert up to 20 lbs. force occasionally, and/or up to 10 lbs. frequently. Prolonged standing, sitting, and walking. Perform repetitive tasks/motions. Must be able to distinguish colors. Must pass fit test for respirator.
