  • Establishes, develops, and maintains excellent referral relationships with referring physicians and discharge planners at targeted acute care facilities.  Performs on-site patient evaluations to determine rehabilitation potential, level of care required and to provide information regarding SJRMC and its scope of services.

    JOB DUTIES

    1.       Actively demonstrates the organization's mission and core values, and conducts oneself at all times in a manner consistent with these values.

    2.       Knows and adheres to all laws and regulations pertaining to patient health, safety and medical information.

    3.       Acts as a resource to all referring parties regarding available services at SJRMC and continuum.  Establishes and maintains excellent relationships with all referring facilities, physicians, and associated clinical staff.

    4.       Serves as a representative of SJRMC to referring physicians and discharge planners in order to identify their needs for rehabilitation services, increase their satisfaction with SJRMC and therefore, improve referral relations.

    5.       Performs on-site patient evaluations to determine rehabilitation potential, appropriate level of care, and determination of patient's nursing needs.  Confers with patient and family to determine their goals and expectations for rehabilitation and provide orientation as appropriate.

    6.       Assess medical stability and nursing needs of inpatient referrals through the intake channel and communications to the SJRMC physicians and associated clinical staff.

    7.       Maintains responsibility for coordination of special patient services on a pre-admission basis such as dialysis, chemotherapy, radiation, special equipment needs, nutritional needs, etc.

    8.       Assists directors in developing and maintaining on-going staff in-service programs.

    9.       Assists directors in the organization of activities to establish and improve relations with referral sources (e.g., off-site marketing presentations, meetings, luncheons, breakfast meetings, group and individual tours, etc.)

    10.   Provides timely and appropriate completion of all pre-admission documentation with prompt and complete communication to nursing units.

    11.   Negotiates any conflicts arising from the referral of a patient's care and ensures a holistic approach to problem solving in order to provide a smooth and efficient transfer process.

    12.   Serves as a clinical liaison to nursing management regarding daily patient admission/transfer/discharge activity.

    13.   Monitors correct assignment of patient types, hospital service codes, and ICD-9 code assignments.

    14.   Acts as a resource to third party payers and provides findings from a patient on-site evaluation as requested.  Includes pre-admission development of patient goals for rehabilitation and negotiation and pre-certification of initial approved length of stay.

    15.   Collaborates with directors and referral development specialist in the assessment of acute care hospitals/facilities in relation to patient referral potential and PM&R consultation services.

    16.   Maintains excellent communication and relations between internal departments that entail frequent interaction (e.g., patient financial services, pharmacy, supply chain management, nursing units, dietary, medical records, etc.)

    17.   Provides ongoing distribution of public relations materials to all potential referral sources.

    18.   Performs other duties consistent with purpose of job as directed. 

     

    JOB SPECIFICATIONS AND CORE COMPETENCIES

    Education:  Knowledge of rehabilitation clinical practice normally acquired by completing a degree in nursing with additional training/certification in rehabilitation.

    Requires graduation from an accredited school of nursing.

    Licensure:  RN Indiana license. Requires a valid Indiana Driver's License.

    Experience:  At least 2 years of rehabilitation nursing experience in order to display a full knowledge of rehabilitation and health care delivery systems.  Must possess knowledge of a hospital organization, patient assessment skills, and rehabilitation medicine as related to patient care and outcomes.

    Other Job Requirements: 

    Excellent interpersonal and communication skills to interact effectively with physicians, clinicians, insurance representatives, patients/families, and a wide variety of potential referrers.  These skills are necessary to effectively represent within the community. 

    Analytical skills to develop plans, goals, and procedures in order to solve complex administrative and referrer/patient problems as well as maintaining a key sensitivity to payer considerations.
