Establishes, develops, and maintains excellent referral relationships with referring physicians and discharge planners at targeted acute care facilities. Performs on-site patient evaluations to determine rehabilitation potential, level of care required and to provide information regarding SJRMC and its scope of services.

Education: Knowledge of rehabilitation clinical practice normally acquired by completing a degree in nursing with additional training/certification in rehabilitation.
Requires graduation from an accredited school of nursing.
Licensure: RN Indiana license. Requires a valid Indiana Driver's License.
Experience: At least 2 years of rehabilitation nursing experience in order to display a full knowledge of rehabilitation and health care delivery systems. Must possess knowledge of a hospital organization, patient assessment skills, and rehabilitation medicine as related to patient care and outcomes.

Excellent interpersonal and communication skills to interact effectively with physicians, clinicians, insurance representatives, patients/families, and a wide variety of potential referrers. These skills are necessary to effectively represent within the community.
Analytical skills to develop plans, goals, and procedures in order to solve complex administrative and referrer/patient problems as well as maintaining a key sensitivity to payer considerations.
