    • Job Summary
      • The Case Manager coordinates the care and reimbursement of an assigned patient population from entry into the hospital through discharge, promotes effective utilization and monitors health care resources to meet legal, regulatory, ethical, and quality guidelines. The Case Manager assumes leadership role within the interdisciplinary team to achieve optimal Clinical outcomes. As the patient progresses through the episode of care the Case Manager will arrange and facilitate post hospital services as needed.
      • Demonstrates population-specific competencies (e.g. age-specific, cultural sensitivity, patient-centered communication) in performance of role.
    • Qualifications Required
      • Education:
        • Bachelor's degree required in Nursing. ADN with 2 years case management experience may be substituted for the BSN degree.
      • Licensure/Certification:
        • RN Licensure in Georgia required. Case Management certification preferred.
      • Experience:
        • 3 years clinical experience as a RN in acute/critical care setting required.

        • Ability to influence decisions, gain cooperation, and facilitate change with all health care providers, patients, and families
        • Demonstrated ability to assess health care needs of patients of specific age groups and cultural backgrounds
        • Principles of Adult Education
        • Good working knowledge of Medicare, QIO, The Joint Commission, Medicaid and other regulatory agency requirements as well as other private third party payor requirements.
        • Good interpersonal and communication skills to effectively communicate with physicians, patients/families, healthcare team, administration, community and third party payers.
        • Respect for and responsibility to protect the confidential nature of the information handled.
        • Ability to work under pressure and adapt to fluctuating workloads.
        • Ability to use independent judgement.
        • Ability to interpret medical record and/or computer terminal information and compare the information with pre-established criteria to analyze data regarding utilization, quality and outcomes.

      • Visual ability to read document, computer terminals
      • Verbal communication skills to speak with staff, patients, visitors, and medical staff
      • Auditory ability to hear staff, visitors, families, and medical staff via phone and in person
      • Manual dexterity to use a PC and software packages, pens, pencils, etc.
      • Ability to move about the organization, campus, and to travel to meetings
      • Cognitive ability to understand and analyze complex health care information and data.
