- The M.D.S./Care Plan Coordinator is a licensed professional nurse responsible for the completion of the Resident Assessment Instrument using information collected systematically from residents, significant others, and other health care professionals associated with the care of the resident. Develops patient care plans and updates plans according to established standards. The M.D.S./Care Plan Coordinator is supervised by the Director of Nursing or designee. Accountability is monitored through patient care conferences, Q.A. activities, and performance appraisals. EDUCATION Graduate of an accredited School of Nursing LICENSURE/CERTIFICATION Current R.N. Licensure in the state of Georgia Basic CPR Certification required EXPERIENCE Basic computer knowledge. Must have experience in MDS/Care plans and have knowledge and understanding of Nursing Home prospective payment system. Knowledge of Nursing home reimbursement mechanisms. Recent experience (within 4 yrs.) as R.N. in Long Term or hospital setting.
