  • The Case Manager/Social Worker- Outreach is primarily responsible for working with diverse populations with multifaceted needs. This will include, but not limited to, assisting clients in following through on established treatment plans; enhancing social functioning; improving the client's quality of life; serving as a planner, advocate, broker, and record keeper for the client.  

    Case Manager-Required Education/Required Training: MASTERS/BACHELORS IN HUMAN SERVICES FIELD PREFERRED/High School Diploma or equivalent PLUS EXPERIENCE REQUIRED. Minimum of 5 years work related experience with a difficult population, preferred.  

    Social Worker-Required Education/Required Training: Masters degree in Social Work from accredited college or university. Licensed or eligibility as a Social Worker (LICSW) required. Required Experience: Minimum 2 years experience in working with chronically mentally ill and/or homeless persons. Minimum 3 years experience in social service management.

    Experience with adolescents; Bilingual Spanish and English strongly preferred. Experience with outreach strongly preferred. Experience working with underserved or homeless populations preferred. * For correctional position-correctional experience- preferred but not required.
