Job Details:
  • The Care Management Support is primarily responsible for various activities that support the clinical care teams. This will include, but not limited, to entering and managing patient related data, as well as patient flow data into the PECS or comparable system; assisting the Care Manager, Nurse Manager, Health Center Director and Medical Director in analyzing trends in patient management.

    Required Education/Required Training: High School Diploma or equivalent. College Coursework in business or health related field preferred.

    Required Experience: Minimum 6 months experience in a medical office. Light typing experience and knowledge of the computer.

    Experience working with underserved or homeless populations preferred. * For correctional position-correctional experience- preferred but not required.
