  • The PharmD will help to implement a comprehensive chronic care model (CCM) for our patients with chronic diseases such as diabetes, hypertension and COPD.  As part of a multi-disciplinary team including physicians, nurses, nutritionists, social services staff, and an optometrist, the PharmD will help to create and implement optimal approaches to team-based patient education, care management and medication therapy management. This position may include oversight of patients on anti-coagulation therapy and patients with polypharmacy.

    Qualifications: 

    Must commit to becoming a Certified Diabetes Educator within the first two years of employment. Graduate of a PGY-1 or PGY-2 pharmacy residency program, ideally with a concentration in Ambulatory Care

    Experience working with low-income populations

    Strong interest in addressing health outcomes disparities

    Bilingual in Spanish or Amharic a plus
