Job Details:
  • The Licensed Practical Nurse - Care Manager is primarily responsible for working with diverse populations with multifaceted needs. This includes, but not limited to, serving as a planner, advocate, broker and record keeper for the patients of the organization; and providing nursing care and program administration, as well as nursing assessments, routine nursing procedures, vein puncture, triage and routine clinical procedures.

    Required Education/Required Training: Graduate of an accredited school of nursing (LPN, AAS, BSN). Licensed or eligibility as LPN In DC. Must have current CPR certification.

    Required Experience: Minimum of 2 years experience in a hospital or ambulatory care environment with experience in medical surgical, emergency room, intensive care, post partum or nursery experience.

    Experience working with underserved or homeless populations preferred. * For correctional position-correctional experience- preferred but not required.
