  • UW Neighborhood Clinics (UWNC) is looking for a Manager of Nursing and Care Management to participate in the development and advancement of the clinical support infrastructure in primary care areas to include care coordination for high risk individuals with chronic illness.

    The Manager for Nursing and Care Management participates in the development and advancement of the clinical support infrastructure in primary care areas to include care management for individuals who are at high risk for poor health outcomes and higher costs.  .  This individual will work with an interdisciplinary team to promote team based care in the primary care setting.  The goal of care management is improvement of patient outcomes, decrease the cost of care in the targeted populations and improvement of the experience of the patient and care team.  A solid theoretical foundation with a creative positive approach to rapid cycle improvements is essential to support program development.


    The Manager actively participates with the leadership team for Nursing and Care Management to develop: 1) strategies to ensure care is coordinated between multiple providers as well as settings; and 2) processes to ensure patients receive necessary acute care appointments to minimize preventable emergency room visits and hospitalizations.  Focus of care management will be on functional assessment of the patient, the patient's understanding and advancement of the disease/state of heath, knowledge of and adherence to medications, warning signs and when to report them to the care team. Care management will be provided in multiple settings; in the office, and at transitions of care


    The Manager is responsible for the clinical practice of the registered nurse care managers throughout system and works as a team with the clinic leadership team to assure quality care for patients.  This role also provides guidance to the Health Navigators as the team member who support the care team.  The Manager as a member of the Nursing and Care Management leadership team implements, coordinates and evaluates services of the RN Care Managers.  The Manager must be effective in developing and maintaining strong collaborative relationships with primary care department managers, physicians and staff as well as others within UW Medicine, the health care system and community.


    The ideal candidate will have a Bachelors of Nursing, Masters of Nursing degrees, or Masters Degree in a health related field. Ability to deliver care management services or supervising nurses conducting care management services.  Experience with an EMR, particularly Epic, is highly desirable.
